- Personal Care Assistance: Aids with daily activities such as bathing, dressing, grooming, and toileting, while respecting the client's privacy and dignity.
- Vital Signs Monitoring: Regularly measure and record vital signs, including blood pressure, pulse rate, temperature, and respiration, and promptly report any significant changes to the supervising healthcare professional.
- Medication Support: Assist clients with medication reminders, ensuring adherence to prescribed medication schedules. Document and report any concerns or issues related to medication administration.
- Mobility and Transfers: Help clients with mobility needs, including transferring from bed to chair, wheelchair mobility, and assistance with ambulation, always ensuring their safety.
- Assist with feeding as necessary.
- Companionship and Emotional Support: Foster a warm and supportive relationship with clients, engaging in conversation, providing companionship, and actively listening to their needs and concerns.
- Documentation and Reporting: Maintain accurate and up-to-date records of client care activities, including observations, interventions, and changes in health status. Report any significant findings to the appropriate healthcare professionals.
- Collaborative Approach: Work closely with the healthcare team, including registered nurses, physicians, and therapists, to ensure effective coordination of care and address client-specific needs.
- Client and Family Education: Educate clients and their families on proper care techniques, disease management, and self-care strategies to enhance their overall well-being and independence.
- Protects organization's value by keeping patient information confidential and complying with HIPAA regulations.
- Serves and protects the hospital community by adhering to professional standards, hospital policies and procedures, federal, state, and local requirements, and JCAHO standards.
- Updates job knowledge by participating in educational opportunities; reading professional publications; participating in professional organizations; maintaining certifications.
- Uses PPE where appropriate.
- Does not arrive late to work (more than 5 minutes past the start of a shift) and maintains an attendance record which is within the standards required by the facility/ company.
- Perform other duties as assigned.
- One year recent Home Health experience required; more experience preferred.
- Current State Certification in good standing
- Current BLS Certification
- Must have a valid, unrestricted driver’s license
- Must have valid auto insurance
- Other certifications or licenses may be required for this position
- Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills
- Physical Abilities: Remain in a stationary position, move about, move equipment (50-100lbs), pushing, pulling, bending.
- Interpersonal Skills - to work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and a professional appearance.
- Technical/Motor Skills - Ability to grasp, fine manipulation, pushing/pulling and able to move about when assisting with procedures and/or using department equipment.
- Mental Requirements - Must be able to cope with frequent contact with the general public, customers, and meeting deadlines under pressure. Must be able to work under close supervision occasionally as well as working without assistance from other personnel. Must be able to contend with irregular activity schedules occasionally and continuous concentration to detail.
- Sensory - Visual acuity, ability to effectively communicate
- Basic patient care skills are necessary. Must have a valid CNA certification in the state where they will be practicing and BLS certification.
