Endorsement Process
Hawaii uses an online portal endorsement process for respiratory therapists already licensed in another state. You'll need to submit your completed application along with supporting documents and the $340 application fee.

Where fingerprinting applies, submit your authorization as early as possible, since background check processing is one of the most common sources of delay.
Jurisprudence Exam Required: Hawaii requires a mandatory Respiratory Therapy Jurisprudence Examination covering Hawaii-specific law. This cannot be waived regardless of your experience level or compact status. Schedule this early — no license (temporary or permanent) will issue until you've passed it.

Temporary License

Renewal Process

- Renewal window: Opens renewals accepted approximately May 4 through June 30 of the renewal year (most recently May 4, 2026 — June 30, 2026 for the 2026 triennial renewal)
- Late penalty: Forfeited licenses may be restored within 1 year of expiration upon payment of renewal + restoration (REAC) + CRF fees ($120 Renewal + $150 CRF + $12 REAC = $282 for inactive reactivation), and failure to restore within 1 year terminates the license and requires reapplication as a new applicant
- Traveler tip: Track your Hawaii license expiration date carefully — a lapsed license can disrupt active assignments
