Overview
Hawaii is NOT an OT Compact member. Occupational therapists must obtain a Hawaii-specific endorsement license before practicing. No multistate license option is available; each assignment requires independent Hawaii licensure.
Endorsement Process
Hawaii uses an online portal endorsement process for occupational therapists already licensed in another state. You'll need to submit your completed application along with supporting documents and the $279 application fee.
- Submit endorsement application via Hawaii board portal
- Request license verification from current state (via NBCOT or direct) (can run concurrently)
- Complete any state-specific requirements (jurisprudence exam, background check, etc.) (can run concurrently)
- Await board review and license issuance
License verification is handled through NBCOT Verification of Certification, sent directly from NBCOT to the Hawaii Occupational Therapy Program (original or electronic accepted). Hawaii requires verification from every state where you hold or have held an occupational therapist license, so request verification early to avoid delays.
Background check requirements vary by applicant — confirm the current fingerprinting and screening steps with the Hawaii Board of Occupational Therapy. Where fingerprinting applies, submit your authorization as early as possible, since background check processing is one of the most common sources of delay.
Jurisprudence Exam Required: Hawaii requires a mandatory Occupational Therapy Jurisprudence Examination covering Hawaii-specific law. This cannot be waived regardless of your experience level or compact status. Schedule this early — no license (temporary or permanent) will issue until you've passed it.
After all components are received, the Board reviews applications on a rolling basis. Check the Hawaii Board of Occupational Therapy website for status updates on your application.

Travelers should factor these licensing costs into their assignment budgeting. Many staffing agencies, including Fusion Medical Staffing, offer licensure reimbursement programs to help offset these expenses.
Temporary License
Hawaii does not issue temporary occupational therapy licenses. Your permanent license must be fully issued before you can begin working on assignment. Travelers should plan their start dates accordingly and allow the full processing window before their expected first day.
Processing Time
Hawaii occupational therapist endorsement applications typically take Approximately 15-20 business days (about 3-4 weeks) for a complete application to process. The timeline is subject to moderate variability depending on the completeness of your application and external factors.
In the best-case scenario, processing can be as fast as 3-4 weeks if application submitted immediately before board meeting, all documents complete, and prior state verification already obtained.
Common sources of delay:
- Hawaii Board of Occupational Therapy meets on a fixed schedule; applications submitted between meeting dates experience 2-4 week queue delays before initial review
- State-specific background check processing can add 2-3 weeks if board requires additional fingerprinting or FBI clearance beyond standard verification
- NBCOT verification requests routed through board mail/portal system rather than real-time API, causing 1-2 week delays in license verification receipt
Hawaii does not offer an expedited processing track. The best way to minimize your wait time is to ensure all application components — fingerprints, verification, and any required exams — are completed before or immediately after submitting your application.

Renewal Process
- Renewal cycle: Every 2 years — $186 via Hawaii Board of Occupational Therapy portal
- Renewal timing: Expiring December 31st of even-numbered years.
- Late penalty: restoration (late renewal) of a forfeited license is $261 active / $87 inactive for both OT and OTA, available within 1 year of forfeiture, and after 1 year the license terminates and the person must reapply as a new applicant
- Traveler tip: Track your Hawaii license expiration date carefully — a lapsed license can disrupt active assignments
